Pterional Craniotomy and MCA Aneurysm Clipping – Overview for Surgeons
- Divide veins bridging Sylvian fissure.
- Apply temporary clip to reduce blood flow to aneurysm, then apply permanent clip to neck of aneurysm.
Laparoscopic Nissen Fundoplication – Suture Pattern for Surgeons
- Care is taken to avoid the vagus nerve during dissection.
- Esophageal hiatus is closed with sutures and pledgets prior to fundoplication, narrowing the hiatus to approximately 2.5 cm in diameter.
- A 56-French bougie is placed across the gastroesophageal junction.
- A Penrose drain is used to retract the esophagus anteriorly.
- Fundus is wrapped posteriorly.
- Sutures with pledgets are placed, connecting the two portions of the fundus, with the superior suture anchored to the esophagus.
